     Member and Leader enrollments are not official until they are accepted by the Cooperative Extension Office.

    - Mohave County 4-H Project codes:

    - Member Enrollment form:     Fillable Member Enrollment Form:   Type, Print and Mail.

    - Leaders Yearly Enrollment form:     Fillable Leader Enrollment Form:   Type, Print and Mail.

          Leader Enrollment Forms are for Certified Volunteers, and must be completed each year with new enrollments.

     NEW LEADERS must complete a Leader Application form and Personal references and interview.
