Could your sod be too wet driving out oxygen? No roots can live tithout air. 
Let it dry out between irrigation. Dig out a clump of grass and inspect the 
roots and soil for  disease or organisms feeding on the roots. Are you 
fertilizing? Over fertilizing? Is the remaining grass healthy in appearance?
