Documents and Forms

Following are the most commonly used Yavapai County Master Gardener documents and forms (pdf format).

 

Event Sign-in Form (pdf file) is to be filled out for the University of Arizona for affirmative action reporting purposes. Master Gardeners who give presentations to groups of adults are responsible for having the participants sign the form. The completed form should be mailed to the Prescott Cooperative Extension Office, 840 Rodeo Dr, Bldg C, Prescott, AZ 86305, Attention Master Gardener Volunteer Coordinator. Note: If a presentation is made to a group of children, an e-mail containing information on the event, location, and number in attendance should be sent to the Volunteer Coordinator.

Hours Reporting Form (pdf file) includes mailing / faxing instructions. To report hours electronically, go to the Report Volunteer & Continuing Ed Hours page.

Master Gardener Association ByLaws (pdf file)

Master Gardener Association Committees (pdf file) and their responsibilities.

Program Policy (pdf file) contains the guidelines established by the Yavapai County Extension Director for the Master Gardener Program.

Terms of Agreement (pdf file) contains terms of compliance between Yavapai County Master Gardeners and the University of Arizona. A signed copy must be on file for all Yavapai County Master Gardeners.

Volunteer Information Sheet (pdf file) contains information on areas of interest, emergency contact, etc. Copies are kept on file for all Master Gardeners. The information is used to match Associate Master Gardeners to mentors with similar interests, to identify candidates with specific skills
