A Lygus management survey was developed by the RAMP Evaluation team (Fournier, Ellsworth, Goodell, Parajulee & Bundy) to measure changes in Lygus management practices, knowledge, and use of available resources. Survey instruments are provided below.

- Lygus Survey Part B (Sampling) (PDF, 62KB) Page 3, includes questions on Lygus sampling procedures and thresholds, based on Arizona standards. Questions may be modified to measure state-specific thresholds and sampling procedures.
- Lygus Survey Part C (Resources) (PDF, 96KB) Pages 4-7, includes questions on use of Lygus management resources in Arizona, California and Texas as well as generic resources. Form may be shortened to include only generic resources and resources for your state.
- Cover Sheet (PDF, 82KB) To be submitted with completed surveys to Al Fournier.
- Lygus Survey Presentation (PowerPoint, 153KB) Generic version of PowerPoint presentation. This can be modified for use in any participating state.
- Disclosure Form (PDF, 415KB) A disclosure form must be provided to each potential survey participant.
- Informed Consent Script (PDF, 56KB) A brief statement explaining the voluntary nature of the survey which can be read to potential participants.
